NOTETAKER APPLICATION |
Please read the following statements and rules carefully.
For reasons of Confidentiality, Student Disability Services will not give the name of the student to the notetaker. Student Disability Services will give the notetaker a code name for the student who will receive the notes, not the student's name. The code name for the student should be at the top of the first page of each day's notes.
The course prefix and number (example: MAC 1104) or the course title (example: College Algebra) should be at the top of every page of notes.
The date of the class meeting should be at the beginning of each day's notes.
Number note pages sequentially (1,2,3,...21, 22, 23,...) throughout the semester.
Write notes in complete sentences whenever possible and appropriate. These notes are used by students with hearing impairments to supply the information they did not hear in class, or by students with learning disabilities to organize and complete the information they missed in class.
E-mail notes to sds@mail.ucf.edu or fax to phone (407) 823-2372. The notetaker should always keep a copy of the notes.
Notes also may be photocopied free of charge at Student Disability Services, Ferrell Commons Room 132. Write legibly in black or dark blue ink or type the notes so the notes can be photocopied.
Notes should be e-mailed to Student Disability Services within 24 hours or the class (e-mail notes immediately after class when a test is scheduled for the next class period). This will give the student time to read the notes to prepare for the next class meeting.
The notetaker must attend every class and arrange for another student to take notes if absent from class.
To be paid for notetaking, the notetaker must sign an "Agreement to Purchase Class Notes" at the
beginning of the semester in Student Disability Services. Please do not provide notes for the student until you have signed an
"Agreement to Purchase Class Notes."
The notetaker also must sign the "Notetaker Invoice" at the end of the semester in Student Disability Services. Your check will be mailed to you
at the address on the "Agreement to Purchase Class Notes" several weeks after the end of the semester.
